bfe899b126 Some more map extractor fixes. Patch provided by andstan.
Coding style fix.
Removed hack from lava detection code.
Please test water/lava detection. Should work fine now.

Also now gm not allowed logout instant in case fight/duel to prevent unexpected death.

9fc7dd9ead [6853] [2008_11_27_01_mangos_playercreateinfo_item.sql] Use DBC data for creating initial character items.
Thanks to WCell team for DBC structure research.
`playercreateinfo_item` content removed from mangos.sql and table can be used now only for custom additional initial items.
